ABOUT THE PROGRAM
About the Program
Community Healthcare Network (CHN) Health Home Care Management is a free program for people with Medicaid who need help managing their healthcare and connecting to services in the community.
Each member is partnered with a dedicated Care Management team that provides personalized support, advocacy, and care coordination.
We help individuals remain healthy, connected, and independent by coordinating care and connecting members to the services and resources they need to thrive in their communities.
Who can join?
How Your Care Management Team Can Help
We Support Participants Who Need:
- Adults age 21 and older with active, unrestricted Medicaid
- Have two or more chronic health conditions, HIV, Serious Mental Illness (SMI), Sickle Cell Disease, physical disabilities, or other qualifying health needs
- Need support navigating healthcare and community resources
- Could benefit from Health Home Care Management or Home and Community-Based Services (HCBS)
- Connect you to primary care, specialty care, and behavioral health services
- Schedule and manage medical appointments
- Coordinate home care services and long-term support resources
- Assist with housing, food, transportation, benefits, and Medicaid resources
- Connect you to mental health and substance use treatment services
- Support transitions after hospitalizations and emergency room visits
- Support successful transitions from incarceration back into the community
- Provide advocacy and care coordination across healthcare and social service systems
- Help identify eligibility for Health Home Care Management and HCBS services
- Help managing multiple chronic health conditions and complex healthcare needs
- Assistance in accessing healthcare, behavioral health, and community resources
- Care coordination among medical, behavioral health, rehabilitation, specialty care, and long-term support providers
- Support accessing home care, personal care services, durable medical equipment, and mobility supports
- Assistance after frequent hospitalizations or emergency room visits
- Advocacy and navigation to reduce barriers to healthcare and improve health outcomes

Choosing the right Health Home can be as quick as clicking a button! We are partnered with 20 care management agencies in all five boroughs. We also have staff who speak over 10 languages. CHN Health Home is also contracted with 10 of the major managed care plans in New York City. We work closely with them so that we can keep members healthy.
